How to Get Started

New patients are welcome to call our office to request new patient paperwork. Here's what the process looks like:

  1. Call our office at 505-327-4867 to request your first appointment.
  2. Complete your new patient forms ahead of your visit (Sent via text and email prior to appointment).
  3. Bring your ID, insurance card, and a list of any current medications.
  4. Patients must arrive 30 mins early to fill out paperwork (if not early, you will have to reschedule)
  5. If all new patient paperwork is already complete, plan to arrive 15 minutes early for your first visit so we can get you checked in smoothly.

What to Bring to Your First Visit

  • Valid photo ID
  • Insurance card (or FFP+ membership info)
  • List of current medications, including supplements
  • Previous medical records, if available
  • Payment method for any copay or balance

Not insured,? Ask us about FFP+, our flat-rate membership program that covers unlimited visits for one monthly fee.

Frequently Asked Questions

  • Do you see walk-in patients?

     We recommend calling ahead or scheduling online to ensure we can give you the time and attention you deserve. Same-day appointments are often available.

  • How long is a new patient appointment?

    New patient appointments are typically longer than follow-up visits so we have plenty of time to get to know you and your health history. Plan for about 45–60 minutes for your first visit.

  • Do you offer telehealth?

    Yes, telehealth is available for all patients except for new patients. 

  • What if I need a specialist?

    We'll coordinate referrals to trusted specialists when needed. And because we have in-house lab and X-ray, many diagnostic questions can be answered right here before a specialist is ever needed.

  • Do you see children from birth?

    Yes. We welcome patients from newborns to seniors. Pediatric care is a core part of what we do, and we offer a full schedule of well-child visits and childhood vaccines.
