Monthly Adult Plan

Starting at $85 / month

Ideal for individual adults seeking consistent, high-quality primary care and preventive services. For adults under the age of 50 it's $85 monthly. For adults aged 50-64, services are $110 monthly.

Monthly Child Plan

Starting at $15 / month

Ideal for families with one or two children under 18 that may require extra care or visits throughout the year. The $15 monthly rate is added to an adult plan. For children without an adult, it's $60 monthly.

Monthly Family Plan

Starting at $95 / month

Ideal for families with high deductibles or anticipated healthcare needs. One adult and one child starts at $95 monthly. Every additional child is $15, and no additional charge for more than 3 children, or $45 monthly per family plan. The max fee is $170 per family with 3 or more children.
