Monthly Adult Plan

Starting at $80 / month

Ideal for individual adults seeking consistent, high-quality primary care and preventive services. For adults under the age of 50 it's $80 monthly. For adults aged 50-64, services are $105 monthly.

Monthly Child Plan

Starting at $10 / month

Ideal for families with one or two children under 18 that may require extra care or visits throughout the year. The $10 monthly rate is added to an adult plan. For children without an adult, it's $55 monthly.

Monthly Family Plan

Starting at $90 / month

Ideal for families with high deductibles or anticipated healthcare needs. One adult and one child starts at $90 monthly. Every additional child is $10, and no additional charge for more than 3 children, or $30 monthly per family plan. The max fee is $165 per family with 3 or more children.
